The Incident: Incorrect Panel Orientation During Installation

At a mixed-use complex, a distributor’s field service team assisted with installing tinted IGUs for a skybridge. While the product was delivered correctly, the field technician failed to confirm orientation marks. As a result, several units were installed with the coating on the wrong surface—causing visible reflectivity issues and light distortion.

The Chain of Failures

Orientation labels were removed during unpacking, and no backup reference was used.

The field team assumed uniformity instead of checking unit layout.

No visual verification protocol was in place before lifting units into place.

Ten panels had to be removed, reordered, and reinstalled using a crane.

What Could Have Prevented It

Include durable, non-removable orientation markers on glass units.
